The Your Health Matters pilot, a CAREDEEP pilot project delivered as a partnership between Riverside and West Road Medical Practice, has now been running for a year with great success!
The project was created to help improve screening and immunisation uptake within the Czech, Slovak, and Romanian communities at West Road Medical Centre. Alongside this, patients have also received support to use the NHS e-Consult App and access a range of welfare advice and guidance.
In July, we were delighted to receive funding from the Newcastle Fund to continue and expand the pilot. The next phase will see us continuing our work with West Road Medical Centre and working with two more GP practices until March 2026.
We’ll soon be inviting patients from all three practices to the Carnegie Building to take part in sessions about:
✅ Families First support and services
✅ Getting medical advice directly from a pharmacist
✅ Discussions about immunisations and other preventative health measures
We’re excited to keep building on the success of this project and continue supporting our local communities to take charge of their health!
